How to sign up for OhioHealth MyChart

Create an account

There are multiple ways to sign up for MyChart: Instant activation via text or email link (recommended method), registering with your personal information, using an activation code or using an existing MyChart Epic ID.

How to sign up with instant activation using a text or email link (recommended method)

This method should be available for patients who have scheduled an appointment, procedure, checked-in for an appointment, been admitted to or discharged from a care location, or provided a mobile number or email.

  1. Look for a text message or email from OhioHealth inviting you to sign up for MyChart. These links are time-sensitive and will expire after some time. If your link has expired, you can still sign up using another verification method, like those listed below. [show picture of what these look like]

  2. Tap or click the secure link in the message.

  3. Confirm your date of birth to verify it’s you.

  4. Verify your contact information and choose how you want to receive a two-factor authentication code — via email or the phone number on file.

  5. Enter the authentication code.

  6. Choose a username and password for your account.

  7. Once you’ve reviewed and accepted the terms and conditions, your account is active!

How to sign up using your personal information

If you don’t have an activation code or Epic ID, you can enter personal verification information instead.

  1. Go to the OhioHealth MyChart login screen on the OhioHealth MyChart website or mobile app and select Sign up.

  1. Choose the option to Sign up with your information.

  1. Enter your information in the required fields. Then select Next.

  1. Choose how you want to receive a two-factor authentication code — via email or the phone number on file.

  1. Enter the verification code that was sent to your email or phone number to confirm the contact information is your own.

  1. Once you’ve reviewed and accepted the terms and conditions, your account is active!

How to sign up using an activation code

Your activation code may be generated by the front desk staff of your care location when you check in for your appointment or can appear on your After Visit Summary. If you do not receive an activation code, you can call MyChart customer care at (614) 533-6924 to get a new activation code.

  1. Go to the OhioHealth MyChart login screen on the OhioHealth MyChart website or mobile app and select Sign up.

  1. Enter your activation code exactly as it appears. Then select Next.

  1. Enter your date of birth and contact information. Then, choose how you want to receive a two-factor authentication code — via email or the phone number on file.

How to sign up with an existing MyChart Epic ID

Do you already use MyChart but need an account specifically for OhioHealth? If you have a MyChart account already, you can use your existing login information (also called your Epic ID) to sign up for OhioHealth MyChart.

  1. Go to the OhioHealth MyChart login screen on the OhioHealth MyChart website or mobile app and select Sign up.

  1. Select Continue with your Epic ID.

  1. Enter your existing Epic ID (username and password).

  1. Confirm your date of birth to verify it’s you.

Download the OhioHealth MyChart app

Manage your health information from your phone or tablet with the OhioHealth MyChart app, available for iOS and Android.
