No, OhioHealth MyChart is a free service for all OhioHealth patients.

How do I sign up for OhioHealth MyChart?

There are several ways to sign up for OhioHealth MyChart. You could: Have your care team sign you up directly while you’re at the front desk or in the exam room. Receive a MyChart activation code on your “After Visit Summary” or billing statement. Receive a text message or email with an activation code when you come in for a visit. Use self-signup online to create an OhioHealth MyChart account by matching your information against what’s on file in your medical record or with third-party identity verification. What should I do if my activation code doesn’t work?

For your security, your OhioHealth MyChart activation code expires after a set period. It’s also not valid after you use it. If you’re having problems with your code, email MyChartSupport@OhioHealth.com or call the support line at (614) 533-6924 .

I forgot my username or password. What should I do?

If you forget your OhioHealth MyChart username or password, go to the OhioHealth MyChart sign-in page and select Forgot login information? You will go through two-step verification to confirm your identity and recover your username or password.

Can I view a family member’s medical record in OhioHealth MyChart?

Yes, you can view a family member’s record in OhioHealth MyChart if you have proxy access . This type of access lets you see medical records of family members and others you care for with their permission. You must be at least 18 years old to have proxy access to another person’s medical record. You can also give a family member or friend access to your medical record when you need help managing your appointments and other medical needs. To grant access to your medical record, send a proxy invite. Select Sharing Hub from the main menu in OhioHealth MyChart. Then follow the prompts.
