Supplier Registration Portal
To be considered for future opportunities at OhioHealth, please register in OhioHealth’s Supplier Registration Portal. Registering your business is the first step to partnering with OhioHealth. The Supplier Portal is a self-service tool powered by the ConnXus platform. Registration is free and allows registrants to view competitive contracting opportunities posted.

Credentialing Information for Healthcare Suppliers
For the continued safety of our patients, families and staff, we require all suppliers providing services within patient service areas maintain appropriate credentials specific to competency and preventative health.
If you have a signed contract with OhioHealth or have been advised by Environmental Services or Supply Chain to complete the registration process, you must register within Vendormate, our third-party vendor credentialing and access provider.

New Supplier Setup Forms
- ACH Enrollment Form with W-9 - Effective March 1, 2019, the following documents are needed for an ACH vendor:
- New vendor – W9 + OH ACH fillable form and cancelled check or invoice with banking*
- Change to existing vendor – W9 + OH ACH fillable form + cancelled check*
- Alternate acceptable forms are:
- W9 + OH ACH fillable form + invoice with banking
- W9 + bank letter + invoice with banking
- W9 + bank letter + cancelled check

- Credit Card Enrollment Form with W-9
