Access or manage a family member's OhioHealth MyChart

OhioHealth MyChart proxy access lets you view the medical records of your family members and others you care for — with their permission. They can grant you access using MyChart or you can gain access by filling out an authorization form.

What is a proxy?

A proxy is someone authorized to view another person’s medical records in MyChart. Proxies can also take actions on a patient’s behalf, such as sending messages to the care team or requesting a medication refill. The level of access you have as a proxy can vary based on the patient’s age or your relationship to the patient.

Proxies must:

  • Be at least 18 years old.
  • Have their own OhioHealth MyChart account.

How to obtain proxy access to someone else's account

To gain access to another person’s OhioHealth MyChart medical record, you can either fill out and return a form or have them grant you access on their MyChart account.

Complete the authorization form

Review and complete the OhioHealth MyChart authorization for proxy access form. Return the form in-person, by mail or fax to the patient’s OhioHealth care team’s office.

Grant someone else access to your account in MyChart

You can also grant a family member or friend access to your medical records directly in OhioHealth MyChart:

  1. Select Share My Record from the OhioHealth MyChart main menu.
  2. Select Friends and Family Access, then Invite Someone.
  3. Enter the name and email address of the person you’d like to grant access.
  4. Agree to the terms and conditions and select Send Invite.
  5. The person you’ve granted proxy access to now has 24 hours to accept the invitation from their own OhioHealth MyChart account.
