Referral Information
Patients do not need a physician referral to make an appointment with our Spine, Sport & Joint physicians. To make an appointment for physical therapy, you will need a prescription.
Where to Park
Ample, free parking is available in a surface lot in front of the building. Enter the Spine, Sport & Joint Center using the entrance to the right.
What to Bring to Your Spine, Sport & Joint Appointment
- Photo ID
- Insurance card(s)
- Co-payment(s), if required
- Tests results/films requested by physician
- List of medications you are taking
- Medicare pharmacy insurance drug card, if applicable
Please try to arrive 15 minutes prior to your appointment time. If you cannot make your appointment, please give us 24-hour notice, or as soon as possible.
