What should I expect at my ophthalmology appointment?

Please arrive 15 minutes before your scheduled appointment and bring a photo ID, your current insurance card, a payment method for your copayment if required and any past medical records or test results that may help your care team. If you have Medicare, bring your pharmacy insurance drug card.

What if I need to cancel or reschedule?

We will help you reschedule for a better time. Please give 24 hours’ notice if you need to cancel or reschedule an appointment when possible.
