Swing Bed Program
The OhioHealth Swing Bed Program supports patients transitioning from hospital care to home or another care setting.
SWING BED LOCATIONSBridge the gap between hospital and home
We offer short-term rehabilitation and skilled nursing services to help you or your loved one regain independence after an illness, injury or surgery.
What to expect during your stay
Most patients stay one to two weeks in our short-term care facility. During this time, you’ll work with a team of providers, nurses, therapists and dietitians to set and achieve recovery goals. All hospital services remain available, including emergency care.
Therapy services
Based on your diagnosis, a therapist may see you multiple times per day, for up to 45 minutes at a time. Therapy is tailored to your diagnosis and may include personal care activities such as bathing, dressing and kitchen skills. Other activities could include strengthening exercises like walking or stair training.
Important details
- Sessions may begin as early as 7 AM and continue throughout the day until 5 PM.
- Therapy may occur in your room, hallway or rehabilitation department.
- Weekend therapy is reduced to allow for rest and recovery.
- Family meetings with therapists and providers are available and encouraged.
Frequently asked questions
- Who qualifies for swing bed care?
-
You may be eligible for our Swing Bed program if:
- You’ve had a three-day inpatient hospital stay.
- Your attending physician recommends swing bed care.
- You require skilled nursing or therapy services and are expected to make progress.
- What are the benefits of a swing bed program?
-
Our Swing Bed Program bridges the gap between hospital and home and provides education about the next stage of recovery. If you or a loved one would benefit from short-term specialized medical services and rehabilitation following a hospitalization and are not able to be discharged yet, a swing bed unit may be an ideal option.
With the support of a team of providers, nurses, therapists and dietitians, most patients typically return home after an average stay of one to two weeks.
- How do I get started in the program?
-
Once it is determined that you require skilled care, a referral will be made to the skilled swing bed team. The swing bed coordinator will then make arrangements with the interdisciplinary team for your admission to the program.
- What is the difference between the swing bed program and an extended care facility?
-
Both services must meet the same requirements for qualification and admission, but our swing bed program placements tend to be more short-term than an extended care facility placement. If a longer length of stay is necessary, a patient may need to transfer from the swing bed program to an extended care facility.
