Bridge the gap between hospital and home

The transitional care program at Shelby Hospital is for patients who are transitioning from the hospital to home, or from one care setting to another. We assist in recovery by providing a wide range of rehabilitation services that help you or a loved one regain a certain level of independence after an injury, illness or procedure.

Prepare for the next stage of recovery

On average, patients stay at our short-term care facility for one to two weeks. They set goals for returning home to their previous living situation with support from providers, nurses, therapists and dieticians. All of the hospital’s services are available at any time, including those related to emergency care.

Therapy

Therapy

Based on diagnosis, a therapist may see you multiple times per day, for up to 45 minutes at a time. Therapy activities promote personal care, such as bathing, dressing and kitchen activities. Additional therapy could include strengthening exercises, walking or stair training.

  • Therapy may take place in your room, hall or rehabilitation department.
  • Therapy time is reduced on the weekends to promote rest and recovery.
  • Sessions may start as early as 7 AM and continue off and on until 5 PM.
  • Family meetings with therapists and providers are available and part of your care.
Personal care, important items and meals

Personal care, important items and meals

Our team of providers is always available to assist you during your stay. However, they do encourage you to independently perform daily activities.

  • Linen changes are offered daily.
  • Mealtimes are 8 AM, 12 PM and 5 PM.
  • Personal hygiene items are available. You or a family member can bring your own personal products.
  • Personal items should be labeled to avoid being misplaced.
  • Most patients find it helpful to bring five comfortable outfits appropriate for the season.
  • Our facility has a recreational room with games, puzzles and books. You are welcome to bring your own.

Visiting hours

Visiting hours are 8 AM-8 PM. The best time for visiting is during mealtimes or from 5-8 PM, since patients may have therapy throughout the day.
