Community Served by Grady Memorial
OhioHealth Grady Memorial Hospital is located at 561 West Central Avenue, Delaware, Delaware County, Ohio 43015. In developing this Community Health Needs Assessment, we identified the “community served” by Grady Memorial as the residents of Delaware County. See the ZIP codes and communities considered within Delaware County.
We believe that defining the community we serve in this way is supported by where our patients reside. The Ohio Department of Health requires each hospital that is registered in Ohio to file an Annual Hospital Registration and Planning Report by March 1 of each calendar year. A review of the patient origin data from the Annual Hospital Registration and Planning Report for Grady Memorial for 2011 supports the definition of the “community served” as being the residents of Delaware County, Ohio. For 2011, 82 percent of all hospital admissions reside in Delaware County at the time of admission.
It was important to us to get input as to the health needs of our community from those outside of our Hospital. Community input for this report was provided through consultation with stakeholders representing various entities that serve Delaware County. These stakeholders provided information and participated in meetings to identify and prioritize health needs in Delaware County. In the fall of 2012, a meeting of stakeholders convened to kick off a new comprehensive community health planning process and to analyze and prioritize health needs.
