For more than 25 years, the OhioHealth Hospice team has been providing patients and their families with the highest-level of care, helping patients enhance their quality of life and achieve personal goals in the months and weeks near the end-of-life.

OhioHealth Hospice is part of the OhioHealth family of faith-based, not-for-profit hospitals and healthcare services. Because of this relationship, OhioHealth Hospice is able to provide patients and caregivers access to all of OhioHealth’s programs and expert knowledge.

The Hospice program provides continuous support and care for its patients whether they live at home or in a nursing or assisted-living facility. Once a physician has recommended hospice care, our team of professionals will begin creating a care plan based on the patient's and family’s goals and values, while also coordinating all facets of care.

Kobacker HouseKobacker House

Kobacker House is our 24-bed inpatient specialty hospital that combines the best medicine with comfortable amenities and serene, calming surroundings that easily accommodate family and pets. Patients get both the comfort of home with 24-hour medical attention and care. Situated on 10 acres of wooded property, Kobacker House  provides a welcoming environment in a nature preserve setting for short-term care until problems are under control.
