Community Served by Hardin Memorial
OhioHealth Hardin Memorial Hospital is located at 921 East Franklin Street, Kenton, Hardin County, Ohio 43326. In developing this Community Health Needs Assessment, we identified the “community served” by Hardin Memorial as the residents of Hardin County. See the ZIP codes and communities considered within Hardin County.
It was important to us to get input as to the health needs of our community from those outside of our Hospital. Community input for this report was provided through consultation with stakeholders representing various entities that serve Hardin County. These stakeholders provided information and participated in meetings to identify and prioritize health needs in Hardin County. Stakeholder meetings were held from November, 2011 through May, 2012 to analyze the current health care needs in the region. On November 6, 2012, Hardin Memorial hosted a community-wide meeting with community leaders and members representing groups who are medically underserved, low-income, minority, and with chronic disease needs to obtain further community input.
