AOE Update Request Form

Here you can submit updates for an OhioHealth physician's areas of expertise (AOE). Once you've made your additions or deletions, they will be submitted to OhioHealth Credentialing and the Medical Staff Office for review.

The physician's NPI number will call up the physician's current AOE profile.  If you don’t know the NPI number, you can search for it by physician name.
