By accessing and/or using OhioHealth's mobile device software application, you acknowledge that you understand and agree to be bound by the OhioHealth Mobile Device Application End User License Agreement, Privacy Policy, Non-Discrimination Policy, and Terms and Conditions of Use, each of which is set forth below and which may be updated by OhioHealth at any time. Your continued use of the OhioHealth App constitutes your acceptance of each of these policies and terms and any updates thereto.

The OhioHealth App End User License Agreement

This OhioHealth mobile device software application (the "OhioHealth App"), as made available through the application distributor, is licensed, not sold, to you by OhioHealth Corporation ("Licensor" or "OhioHealth"). As used herein, "you" and "your" refer to the individual or entity that wishes to use the OhioHealth App. Your rights to use the OhioHealth App are subject to your prior acceptance of the OhioHealth App End User License Agreement (this "License"), and you agree that the terms of this License will apply to your use of the OhioHealth App. Licensor reserves all ownership and intellectual property rights in and to the OhioHealth App.

The OhioHealth App is supported by your mobile device and certain data networks and enables you to access, review, and use certain data (and perform certain actions with such data) where such data is provided either by Licensor or by a connection to an instance of the Licensor's server edition of MyChart, an application licensed from a third party.

a. Scope of License. Pursuant to the terms of this License, Licensor grants you a non-exclusive, non-transferable, non-sublicensable, revocable limited license to download, install, and use the OhioHealth App on a single mobile device solely for the purpose of performing those functions and tasks and accessing content and services made available to you as an end user of the OhioHealth App solely for your personal, non-commercial purposes, provided that you: (1) are using a mobile device that you own or control, and (2) your mobile device is permitted by the usage rules of the applicable distributor (the "Usage Rules") to access the OhioHealth App. The OhioHealth App also uses third party software licensed to Licensor. All rights not expressly granted herein are expressly reserved by OhioHealth and its licensors. Without narrowing the generality of the foregoing, this License does not allow you to use the OhioHealth App on any mobile device that you do not own or control. Except as may be expressly permitted by applicable law or expressly authorized by the OhioHealth App, you shall not: (i) distribute or make the OhioHealth App available over a network where it could be used by multiple devices at the same time; (ii). rent, lease, lend, sell, redistribute or sublicense the OhioHealth App; (iii). not copy (except as expressly permitted by this License and the Usage Rules), decompile, reverse engineer, disassemble, attempt to derive or access the source code of, modify, or create derivative works of the OhioHealth App, any updates, or any part thereof; .; (iv) use any robot, spider, programmatic, or other automatic device, process, or means to access the OhioHealth App for any purpose, including monitoring or copying any of the material on the OhioHealth App; (v) use any manual process to monitor or copy any of the material on the OhioHealth App, or for any other purpose not expressly authorized in this Agreement, without Company's prior written consent; (vi) frame, mirror, or otherwise incorporate the OhioHealth App or any portion of the OhioHealth App as part of any other mobile app, website, or service; (vii) use the OhioHealth App in any manner that could disable, overburden, damage, or impair the OhioHealth App or interfere with any other party's use of the OhioHealth App; or (ix) remove, delete, alter, or obscure any trademarks or any copyright, trademark, patent, or other intellectual property or proprietary rights notices from the OhioHealth App, including any copy thereof; Any attempt to do any of the foregoing is a violation of the rights of the Licensor and its licensors. If you breach any of these restrictions, you may be subject to prosecution and damages. You are not permitted to use the OhioHealth App for any purpose other than as expressly permitted under this License. You acknowledge that Licensor may audit your use of the OhioHealth App.

The terms of this License will govern any upgrades provided by Licensor that replace and/or supplement the OhioHealth App, unless such upgrade is accompanied by a separate license in which case the terms of that license will govern, and you agree to promptly install any such upgrade and cease use of the prior version. Nothing in this paragraph creates an obligation for Licensor to prepare or provide any upgrades to the OhioHealth App.

b. Consent to Use of Data. You agree that Licensor may collect, store, process, maintain, upload, sync, transmit, share, disclose and use certain data and information, including but not limited to information or data regarding the characteristics or usage of your device, the OhioHealth App as well as personal information, user location data, and user content (collectively "User Data") to facilitate the provision of services or functionality for the OhioHealth App, including, but not limited to authentication, performance optimization, software updates, product support, and other services (if any) to you related to the OhioHealth App or to otherwise improve Licensor's products or to provide services or technologies to you. You acknowledge that use of the OhioHealth App may result in User Data being transmitted between your mobile device and a database server designated by Licensor and that such User Data may be collected, stored, processed, maintained and used by Licensor and/or transmitted or disclosed to or accessed by Licensor's third party licensors and other third parties as Licensor determines is necessary or beneficial to operate the OhioHealth App. More specifically, but without limiting the foregoing, you acknowledge that: (1) information regarding the hardware model and the mobile operating system version of the device on which you are running the OhioHealth App may be collected, transmitted to and stored on a database server designated by Licensor and may be transmitted to Licensor's third party licensors and may be used to make changes, updates or improvements to or to optimize the performance of the OhioHealth App or to otherwise inform future development; and (2) audit logs reflecting your logins, logouts and the activities you have accessed through your use of the OhioHealth App may be generated in connection with your use of the OhioHealth App and may be collected, transmitted and stored on a database server designated by Licensor (and may be made available to Licensors third party licensors) for troubleshooting. BY CLICKING ON THE "ACCEPT" BUTTON, YOU EXPRESSLY CONSENT TO THE FOREGOING COLLECTION, STORAGE, PROCESSING, MAINTENANCE, UPLOADING, SYNCING, TRANSMITTING, SHARING, OR DISCLOSURE OF USER DATA. By continuing to use the OhioHealth App, you indicate your continued consent to such collection, storage, processing, maintenance, uploading, syncing, transmitting, sharing, or disclosure of User Data as well as any collection, storage, transmission and use of data of the type and in the manner described in the OhioHealth App's permission settings on your mobile device, if any, located within the OhioHealth App.

c. Termination. This License is effective until terminated by you or Licensor. Your rights under this License will terminate automatically without notice from the Licensor if you fail to comply with any term(s) of this License. Upon termination of this License, you agree to cease all use of the OhioHealth App, and destroy all copies, full or partial, of the OhioHealth App.

d. Services; Third Party Materials. This OhioHealth App may enable access to Licensor's and third-party services and web sites (collectively and individually, "Services"). Use of the Services may require Internet access and may require that you accept additional end user license agreements and/or terms of service. You understand that by using any of the Services, you may encounter content that may be deemed offensive, indecent, or objectionable, which content may or may not be identified as having explicit language, and that the results of any search or entering of a particular URL may automatically and unintentionally generate links or references to objectionable material. Nevertheless, you agree to use the Services at your sole risk and acknowledge that certain Services may display, include or make available content, data, information, the OhioHealth Apps, websites, or materials from third parties ("Third Party Materials") or provide links to certain third party web sites. By using the Services, you acknowledge and agree that the Licensor is not responsible for examining or evaluating the content, accuracy, completeness, timeliness, validity, copyright compliance, legality, decency, quality or any other aspect of such Third-Party Materials. The Licensor does not warrant or endorse and does not assume and you agree the Licensor will not have any liability or responsibility to you or any other person for any Services, Third Party Materials, or for any other materials, products, or services of third parties. Third Party Materials and links to other web sites are provided solely as a convenience to you. Neither the Licensor, nor any of its content providers, guarantees the availability, accuracy, completeness, reliability, or timeliness of any information or data displayed by any Services or any Third Party Materials.

You agree that any Services may contain proprietary content, information and material that is protected by applicable intellectual property and other laws, including but not limited to copyright, and that you will not use such proprietary content, information or materials in any way whatsoever except for permitted use of the Services.

No portion of the Services may be reproduced in any form or by any means. You agree not to modify, rent, lease, loan, sell, distribute, or create derivative works based on the Services, in any manner, and you shall not exploit the Services in any unauthorized way whatsoever, including but not limited to, by trespass or burdening network capacity. You further agree not to use the Services in any manner to harass, abuse, stalk, threaten, defame or otherwise infringe or violate the rights of any other party, and that the Licensor is not in any way responsible for any such use by you, nor for any harassing, threatening, defamatory, offensive or illegal messages or transmissions that you may receive as a result of using any of the Services. You agree to comply with any applicable third party terms of agreement when using the OhioHealth App (e.g., you must not be in violation of a wireless data service agreement when using the OhioHealth App). The OhioHealth App distributor is not responsible for any third party claims that the OhioHealth App or your possession and use of the OhioHealth App infringes that third party's intellectual property rights.

In addition, third party Services and Third-Party Materials that may be accessed from, displayed on or linked to from the mobile device are not available in all languages or in all countries. The Licensor makes no representation that such Services and Third-Party Materials are appropriate or available for use in any particular location. To the extent you choose to access such Services or Third-Party Materials, you do so at your own initiative and are responsible for compliance with any applicable laws, including but not limited to applicable local laws. The Licensor, and its licensors, reserve the right to change, suspend, remove, or disable access to any Services or Third-Party Materials at any time without notice. In no event will the Licensor be liable for the removal of or disabling of access to any such Services or Third-Party Materials. The Licensor may also impose limits on the use of or access to certain Services or Third-Party Materials, in any case and without notice or liability. Licensor provides limited technical support to you or any end user. Technical support can be reached at 1 (614)533-6924 or at digitalsupport@ohiohealth.com Licensor and you acknowledge that the OhioHealth App distributor has no obligation whatsoever to furnish any maintenance and support services with respect to the OhioHealth App.

e. NO WARRANTY. YOU EXPRESSLY ACKNOWLEDGE AND AGREE THAT USE OF THE OHIOHEALTH APP AND YOUR RELIANCE ON THE OPERATION, OUTPUT OR RESULTS OF THE OHIOHEALTH APP IS AT YOUR SOLE RISK AND THAT THE ENTIRE RISK AS TO SATISFACTORY QUALITY, PERFORMANCE, ACCURACY AND EFFORT IS WITH YOU. TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW, THE OHIOHEALTH APP AND ANY SERVICES PERFORMED OR PROVIDED BY OR THROUGH THE OHIOHEALTH APP ("THE OHIOHEALTH APP SERVICES") ARE PROVIDED "AS IS" AND "AS AVAILABLE", WITH ALL FAULTS AND WITHOUT WARRANTY OF ANY KIND, AND LICENSOR HEREBY DISCLAIMS ALL WARRANTIES AND CONDITIONS WITH RESPECT TO THE OHIOHEALTH APP AND ANY THE OHIOHEALTH APP SERVICES, EITHER EXPRESS, IMPLIED OR STATUTORY, INCLUDING, BUT NOT LIMITED TO, THE IMPLIED WARRANTIES AND/OR CONDITIONS OF MERCHANTABILITY, OF SATISFACTORY QUALITY, OF FITNESS FOR A PARTICULAR PURPOSE, OF ACCURACY, OF QUIET ENJOYMENT, AND NON-INFRINGEMENT OF THIRD PARTY RIGHTS. LICENSOR DOES NOT WARRANT AGAINST INTERFERENCE WITH YOUR ENJOYMENT OF THE OHIOHEALTH APP, THAT THE FUNCTIONS CONTAINED IN, OR THE OHIOHEALTH APP SERVICES PERFORMED OR PROVIDED BY, THE OHIOHEALTH APP WILL MEET YOUR REQUIREMENTS, THAT THE OPERATION OF THE OHIOHEALTH APP OR THE OHIOHEALTH APP SERVICES WILL BE UNINTERRUPTED OR ERROR-FREE, OR THAT DEFECTS IN THE OHIOHEALTH APP OR THE OHIOHEALTH APP SERVICES WILL BE CORRECTED. NO ORAL OR WRITTEN INFORMATION OR ADVICE GIVEN BY LICENSOR, ITS AUTHORIZED REPRESENTATIVE, OR LICENSOR'S THIRD-PARTY LICENSORS SHALL CREATE A WARRANTY. SHOULD THE OHIOHEALTH APP OR THE OHIOHEALTH APP SERVICES PROVE DEFECTIVE, YOU ASSUME THE ENTIRE COST OF ALL NECESSARY SERVICING, REPAIR OR CORRECTION. SOME JURISDICTIONS DO NOT ALLOW THE EXCLUSION OF IMPLIED WARRANTIES OR LIMITATIONS ON APPLICABLE STATUTORY RIGHTS OF A CONSUMER IF YOU ARE A RESIDENT OF SUCH A STATE, THE ABOVE EXCLUSION AND LIMITATIONS MAY NOT APPLY TO YOU. IN THE EVENT OF ANY FAILURE OF THE OHIOHEALTH APP TO CONFORM TO ANY APPLICABLE WARRANTY, YOU MAY NOTIFY THE OHIOHEALTH APP DISTRIBUTOR, AND THE OHIOHEALTH APP DISTRIBUTOR WILL REFUND THE PURCHASE PRICE FOR THE OHIOHEALTH APP TO YOU, IF ANY. TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW, THE OHIOHEALTH APP DISTRIBUTOR WILL HAVE NO OTHER WARRANTY OBLIGATION WHATSOEVER WITH RESPECT TO THE OHIOHEALTH APP. THE OHIOHEALTH APP DISTRIBUTOR SHALL HAVE NO WARRANTY OBLIGATION WHATSOEVER WITH RESPECT TO THE PROGRAM AND NO OBLIGATION TO ADDRESS ANY OF YOUR CLAIMS OR CLAIMS OF ANY THIRD PARTY RELATING TO THE OHIOHEALTH APP OR YOUR POSSESSION AND/OR USE OF THE OHIOHEALTH APP, INCLUDING, BUT NOT LIMITED TO: (I) PRODUCT LIABILITY CLAIMS; (II) ANY CLAIM THAT THE OHIOHEALTH APP FAILS TO CONFORM TO ANY APPLICABLE LEGAL OR REGULATORY REQUIREMENT; AND (III) CLAIMS ARISING UNDER CONSUMER PROTECTION OR SIMILAR LEGISLATION.

f. Limitation of Liability. TO THE EXTENT NOT PROHIBITED BY LAW, IN NO EVENT SHALL LICENSOR BE LIABLE FOR PERSONAL INJURY, OR ANY INCIDENTAL, SPECIAL, INDIRECT OR CONSEQUENTIAL DAMAGES WHATSOEVER, INCLUDING, WITHOUT LIMITATION, DAMAGES FOR LOSS OF PROFITS, LOST OR MODIFIED DATA, BUSINESS INTERRUPTION OR ANY OTHER COMMERCIAL DAMAGES OR LOSSES, ARISING OUT OF OR RELATED TO YOUR USE OR INABILITY TO USE THE OHIOHEALTH APP, THE OHIOHEALTH APP SERVICES, SERVICES, OR THIRD PARTY MATERIALS HOWEVER CAUSED, REGARDLESS OF THETHEORY OF LIABILITY (CONTRACT, TORT OR OTHERWISE) AND EVEN IF LICENSOR HAS BEEN ADVISED OF THE POSSIBILITY OF SUCH DAMAGES. SOME JURISDICTIONS DO NOT ALLOW THE LIMITATION OF LIABILITY FOR PERSONAL INJURY, OR OF INCIDENTAL OR CONSEQUENTIAL DAMAGES. IF YOU ARE A RESIDENT OF SUCH A STATE, THIS LIMITATION MAY NOT APPLY TO YOU. IN NO EVENT SHALL LICENSOR'S TOTAL LIABILITY TO YOU FOR ALL DAMAGES EXCEED THE AMOUNT OF FIFTY DOLLARS ($50.00). THE FOREGOING LIMITATIONS WILL APPLY EVEN IF THE ABOVE STATED REMEDY FAILS OF ITS ESSENTIAL PURPOSE.

g. Export Restrictions. You may not use or otherwise export or re-export the OhioHealth App except as authorized by United States law and the laws of the jurisdiction in which the OhioHealth App was obtained. In particular, but without limitation, the OhioHealth App may not be exported or re-exported (a) into any U.S. embargoed countries or (b) to anyone on the U.S. Treasury Department's list of Specially Designated Nationals or the U.S. Department of Commerce Denied Person's List or Entity List. By using the OhioHealth App, you represent and warrant that you are not located in any such country or on any such list. You also agree that you will not use the OhioHealth App for any purposes prohibited by United States law, including, without limitation, the development, design, manufacture or production of nuclear, missiles, or chemical or biological weapons.

h. Geographic Limitations. The content and Services assessable through your use of the OhioHealth App are based in the state of Ohio in the United States and provided for access and use only by persons located in the United States. You acknowledge that you may not be able to access all or some of the content and Services outside of the United States and that access thereto may not be legal by certain persons or in certain countries. If you access the content and Services from outside the United States, you are responsible for compliance with local laws.

i. U.S. Government Users. The OhioHealth App and related documentation are "Commercial Items", as that term is defined at 48 C.F.R. §2.101, consisting of "Commercial Computer Software" and "Commercial Computer Software Documentation", as such terms are used in 48 C.F.R. §12.212 or 48 C.F.R. §227.7202, as applicable. Consistent with 48 C.F.R. §12.212 or 48 C.F.R. §227.7202-1 through 227.7202-4, as applicable, the Commercial Computer Software and Commercial Computer Software Documentation are being licensed to U.S. Government end users (a) only as Commercial Items and (b) with only those rights as are granted to all other end users pursuant to the terms and conditions herein. j. Licensor. The Licensor of the OhioHealth App is OhioHealth Corporation and is located at: 3430 OhioHealth Parkway, Columbus, OH, 43202. Any legal inquiries may be sent to OhioHealth Corporation, Attn: Office of the General Counsel, 3430 OhioHealth Parkway, Columbus, Ohio 43202.

k. Third party beneficiary. You acknowledge and agree that the OhioHealth App distributor and its subsidiaries are third-party beneficiaries to this license, and that, upon your acceptance of this License, the OhioHealth App distributor will have the right (and will be deemed to have accepted the right) to enforce the terms of this License against you as a third party beneficiary thereof.

l. Choice of law; venue. The laws of the State of Ohio, excluding its conflicts of law rules, govern this License, its construction, and your use of the OhioHealth App. Any action arising under this License shall be brought exclusively in Ohio, and you and Licensor expressly consent to the personal jurisdiction of the state and federal courts located in Ohio for such purposes. Your use of the OhioHealth App may also be subject to other local, state, national, or international laws.

Privacy Policy

Overview

OhioHealth takes very seriously its obligation to protect the confidentiality of your personal information. The OhioHealth mobile device application for consumers (the "OhioHealth App") is intended to connect to servers and systems operated and maintained by OhioHealth in order to provide you mobile access to those systems and to your health information. The OhioHealth App may also contain components with content hosted by third parties.

This Privacy Policy

This Privacy Policy describes how the OhioHealth App uses, stores, and transmits information and data. By using he OhioHealth App to connect to OhioHealth, you consent to this Privacy Policy. OhioHealth may modify this Privacy Policy at any time effective upon its posting. Your continued use of the OhioHealth App constitutes your acceptance of this Privacy Policy and any updates. Your use of the OhioHealth App is subject to the End User License Agreement and, if you chose to use MyChart, you are subject to the MyChart Terms and Conditions. Your access or use of any third party materials, services, and/or websites through the OhioHealth App may require acceptance of additional End User License Agreements and/or terms of service from those third parties. In addition, by accessing or using MyChart, you understand that OhioHealth may use and/or disclose your Protected Health Information as described in the OhioHealth Notice of Privacy Practices, available here.

Purpose

The purpose of this Privacy Policy lets you know what information we collect about you when you use the OhioHealth App, how that information is used, and the ways in which we use the additional information you provide.

Summary

The OhioHealth App may interact with your sensitive data to provide certain features, such as video visits or mobile appointment check-in. The first time you try to use any of these features, we will ask for your consent within the app and will only allow you to use a feature if you give consent. You do not have to provide consent if you do not want to allow the OhioHealth App to interact with your data as requested. The OhioHealth App is developed by Epic Systems Corporation; please refer to Epic's Mobile App Privacy Policy for Patients for more detailed information about the limited ways they may interact with your information to make your use of the OhioHealth App possible.

The OhioHealth App may offer location-based check-in for in-person appointments or allow you to find healthcare providers near you. The first time you try to use any features that use your location, we will ask for your consent within the app and will only access your location if you give consent. You do not have to provide consent if you do not want to allow the OhioHealth App to use your location. We do not store your location data.

We may share information collected through the OhioHealth App with third parties as described below.

In addition to standard MyChart Mobile functionality, the OhioHealth App allows patients to access their electronic health record (EHR). See the section entitled "MyChart App Permissions" below.

Your Personal Information

The OhioHealth App and The Ways in Which OhioHealth Uses Your Information

OhioHealth does not sell or license any information that it may collect from you from using the OhioHealth App. OhioHealth attempts to minimize the amount of your personal or health information stored or retained on your device. Nevertheless, the OhioHealth App may:

Temporarily store your personal information in memory or on the device while you use the OhioHealth App.
Share some limited amount of information for purposes of troubleshooting and error correction directly or indirectly with OhioHealth with your consent.

In addition, in order to provide you certain features, the OhioHealth App may request information from servers and systems owned or operated by OhioHealth or its third party vendors, and those servers and systems may record technical information about that request such as an IP address. The OhioHealth App will also request one or more times your permission to turn on Location Based Services, enabling you to better locate care facilities and doctors near you and more easily arrange transportation to a care facility, should you chose to access this service. technical information about that request such as an IP address. Use of The OhioHealth App

Your use of the OhioHealth App is subject to OhioHealth's privacy policies. You understand that while connected, or attempting to connect, to OhioHealth, OhioHealth may collect, store, process, maintain, upload, sync, transmit, share, disclose and use certain data and related information, including but not limited to information or data regarding the characteristics or usage of your device, system and the OhioHealth App software, and peripherals as well as your personal information, location data, and other content. To use MyChart features in the OhioHealth App, you must have a MyChart account. You can set Apple's Touch ID. or Google's Fingerprint Authentication to log in to your MyChart account. Doing so will let anyone with a fingerprint registered on your device log in to your MyChart account. You can set up Apple's Face ID® to log in to your MyChart account. Doing so will let anyone with a face registered on your device log in to your MyChart account. For additional information regarding MyChart, please review the MyChart Terms and Conditions.

Location Permissions on Android

Your location information is utilized in the OhioHealth app to better provide accurate search results for providers near you and locations. We utilize coarse location and Wi-Fi preferences from your phone which gives your approximate location to a provider or location.

MyChart App Permissions

The OhioHealth App embeds the MyChart Feature/Code Module so patients can access MyChart activities and EHR data. MyChart supports video visits (which require camera and microphone permissions and may require additional third-parties), file attachments (requires storage/read-write permissions), Bluetooth (for device pairing / beacon arrival scenarios), and location (for arrival detection and nearby provider/ED suggestions). Each of those is a separate OS permission and MyChart presents prominent disclosure text explaining how each is used.

Proxy Access

With your permission, your health information available in OhioHealth may be accessed by a person or persons you select. We call this granting "proxy access" to your MyChart account. For additional information on proxy access to your MyChart account, please review the MyChart Terms and Conditions.

Apple Maps

The OhioHealth App uses Apple Maps API(s) to help you locate OhioHealth care facilities. Please refer to Apple's Privacy Policy https://www.apple.com/legal/privacy/data/en/apple-maps/

Google Maps

The OhioHealth App uses Google Maps API(s) to help you locate OhioHealth care facilities. Please refer to Google's Privacy Policy at bit https://www.google.com/policies/privacy.

Google Analytics

The OhioHealth App uses Google Analytics to collect and process non-personally identifying usage information to help us develop and improve our services. Please refer to "How Google uses data when you use our partners' sites or apps" at https://www.google.com/policies/privacy/partners/.

Uber

The OhioHealth App links to your Uber the OhioHealth App/account to help you arrange transportation to a care facility, should you choose to do so. Please refer to Uber's Privacy Policy at https://www.uber.com/legal/privacy/users/en/.

FreshPaint

The OhioHealth App uses FreshPaint to collect user data, such as interactions and device information, to enhance user experience and app performance. We handle all data in compliance with privacy laws and do not share personal data without consent. Location

MyChart may use your location data to notify front desk staff when you arrive for an appointment or to suggest healthcare providers that are near you. MyChart will not store your location data.

Camera

MyChart may use your camera to take new photos or to capture and transmit video for video visits. You choose if you want to use photos to personalize your account or send them as file attachments when you send messages to your healthcare providers. If you send photos to your healthcare providers, they may store them in your medical record.

Microphone

MyChart may use your microphone to capture audio for video visits. MyChart will not store your audio data.

Storage

MyChart may access your device's storage to read and write files you choose to use in MyChart. These files may be used as file attachments that are sent to your provider, or they may be created from file attachments sent to you from your provider. If you send files uploaded from your device's storage to your healthcare providers, they may store them in your medical record.

Bluetooth

MyChart may access your device's Bluetooth to detect other nearby devices. This information will be used to notify front desk staff when you arrive for an appointment. MyChart will not store your Bluetooth data.

Phone Call

MyChart may use your phone to call phone numbers displayed in the app. MyChart will not store your call history or other call data.

Phone State

Zoom utilizes your phone state to detect if the connection for the video visit is still connected. This information is used only to know if there is a connection and is only used when you are on a video visit.

Wi-Fi Access

Used by one of the google dependencies we use for Find-A-Doctor search by location. It uses it to check for network state, so it knows that it can find location matches based on what the user types of in.

How We Protect Your Personal Information

OhioHealth has implemented a variety of commercially standard encryption and security technologies and procedures to protect your personal information stored in our computer system(s) from unauthorized access. OhioHealth also maintains standard physical and electronic procedural safeguards that limit access to your personal information to our respective employees and contractors (or people working on our behalf and under confidentiality agreements) who, through the course of standard business activities, need to access your personal information.

The OhioHealth App is a convenient way to manage your health information. By accessing and/or using the OhioHealth App, you acknowledge that you understand and agree to be bound by the following Terms and Conditions:

The OhioHealth App is not intended to be used to address medical emergencies. IF YOU ARE EXPERIENCING AN EMERGENCY OR URGENT MEDICAL PROBLEM, GO TO THE NEAREST EMERGENCY ROOM OR CALL 9-1-1.

The OhioHealth App is not a substitute for traditional medical advice, consultation, diagnosis, or treatment.

You must be 14 years of age to use the OhioHealth App.

By using the OhioHealth App, you agree not to do the following:

communicate material that is unlawful, fraudulent, libelous, obscene, defamatory, threatening, harassing, abusive, hateful, or embarrassing to any person or entity
communicate a sexually-explicit image
impersonate another person. I acknowledge the information I share pertains only to me
violate the Children's Online Privacy Protection Act
communicate material that is copyrighted, without the permission of the copyright owner
communicate material that reveals trade secrets, unless I have permission of the owner
communicate material that infringes on any other intellectual property rights of others or on the privacy or publicity rights of others
attempt to disrupt the operation of the OhioHealth App by any method, including through use of viruses, Trojan horses, worms, time bombs, denial of service attacks, flooding or spamming
use the OhioHealth App in any manner that could damage, disable or impair the OhioHealth App or service(s) contained within
use any robot, scraper or other means to access the OhioHealth App for any purpose

OhioHealth reserves complete and sole discretion with respect to the operation of the OhioHealth App. OhioHealth may, among other things withdraw, suspend, or discontinue any functionality or feature of the OhioHealth App. Subject to applicable law, OhioHealth reserves the right to maintain, delete, or destroy all communications and materials posted or uploaded to the OhioHealth App and/or service contained within, pursuant to OhioHealth's internal record retention and/or destruction policies.

If you choose to access and/or use MyChart through the OhioHealth App, you are subject to the MyChart Terms and Conditions available within MyChart. In addition, by accessing or using MyChart, you agree to OhioHealth's Notice of Privacy Practices.

By accessing or using the OhioHealth App, which utilizes Uber to help you arrange transportation to an OhioHealth care facility, you agree to be bound to Uber's Terms of Service.

You understand and agree that the use of the OhioHealth App is entirely at your own discretion. You understand that OhioHealth and its physicians, employees, agents, contractors, and affiliates shall not be liable for any direct, indirect, consequential, special, exemplary, punitive or other monetary damages, fees, fines, penalties or liabilities arising out of or relating to your voluntary use of this the OhioHealth App.

The OhioHealth App, website, and all other websites hosted by OhioHealth and the content contained herein and therein are provided by OhioHealth on an "AS-IS" basis. OhioHealth makes no representations or warranties of any kind, express or implied, as to the operation of the OhioHealth App, or the content, products and/or services included therein.
