MDVIP-Affiliated Primary Care

We have partnered with MDVIP, a national network of primary care physicians who focus on wellness and prevention.

MDVIP-affiliated primary care physicians have more time to get to know you beyond your symptoms, in a relaxed, unrushed setting.  Members enjoy conveniences like same- or next-day appointments with minimal wait times and the ability to contact their doctor after hours and on weekends for more urgent needs.

The cornerstone of MDVIP-affiliated practices is the annual MDVIP Wellness Program, comprised of advanced diagnostic tests shown to help better detect certain risk factors earlier. The program includes a heart disease screening developed with Cleveland HeartLab, as well as assessments for diabetes, bone and respiratory health, fitness, sleep, emotional well-being and more. The results of the annual screenings and tests can help provide a more in-depth picture of your health and aid in designing an action plan to help improve your health now and for the future.

Telehealth Appointments Available

Telehealth appointments are offered for your convenience. As a member in an MDVIP-affiliated practice, you can log into your OhioHealth MyChart account anytime to schedule a convenient telehealth/E-Visit.

Visit Information

Arrive 15 minutes before your scheduled appointment to allow time for any updates to patient files and other necessary paperwork.

If you must cancel your appointment, we’ll be happy to reschedule at a more convenient time. Please provide 24 hours' notice, so we can make the time available to other patients.

What to bring to your appointment: 

  • Photo ID
  • Current insurance card
  • Copayment, if required
  • Tests results or imaging that your care team has requested
  • Past medical records you feel are applicable to your health
  • If you have Medicare, your pharmacy insurance drug card
