ContinuCare Program from Hardin Memorial Hospital
ContinuCare ensures Medicare patients receive the care they need.
The ContinuCare Program from OhioHealth Hardin Memorial Hospital provides short-term, skilled care for Medicare eligible patients as long as they meet Medicare Guidelines for qualification. ContinuCare, also known as "swing bed," fills the gap between hospital and home that most Medicare patients need.
This service is available for those who are better but not ready to return home on their own. If you meet the following criteria, you may speak with your physician to be admitted into our ContinuCare Program.
Criteria/Qualifications for ContinuCare:
- Have stayed three (3) consecutive days in any hospital
- Program must be ordered by attending doctor
- Must be receiving skilled services and must be able to make progress
