Take Charge of your Health Information

Your hospital stay or emergency department visit report will help you in many ways to take charge of your health.

 You Can:

  • Review your discharge instructions as a reminder of what you should do next to continue your recovery or follow-up with the next recommended steps
  • Share a copy with a family member or caregiver so they can help you follow the discharge instructions
  • Print and take a copy with you to your follow-up physician visits
  • Keep a record of your hospital visit for future reference

Q: What is MyRecord?

A: MyRecord offers you personalized and secure online access to a summary of your OhioHealth hospital visit.

When you log on to your MyRecord account, you can read, download, print, and send your health summary report. Depending on the type of healthcare services you received, this report may contain:

  • Reason for you visit
  • Summary of the care you received
  • Procedures and results
  • Discharge instructions

Q: Is there a fee to use MyRecord?

A: No. MyRecord is a free service offered to our patients.

Q: When will my health summary report be available? 

A: Your information will be available in your MyRecord account within 36 hours after you are discharged. Please note that not all information may be available in this report.

Q: What if I want access to my full medical record?

A: If you would like a copy of your medical record, you may contact the OhioHealth Health Information Management department or the location where you received your medical care.

Q: If I believe that some of my health information on MyRecord is not correct, what should I do?

A: Your MyRecord information comes directly from your OhioHealth electronic medical record. To request a correction, you may contact the OhioHealth Health Information Management Department or the location where you received your care. Please see additional information under “Changes to Your Protected Health Information” in the OhioHealth Notice of Privacy Practices.

Q: Can I view a family member's health record in MyRecord?

A: No. This functionality is not available in MyRecord.

Q: Can my child have a MyRecord account?

A: Patients must be 18 years of age or older to have a MyRecord account.

Q: Can my spouse and I share one MyRecord account? 

 A: No, due to the sensitive nature of medical information, each adult must establish his or her own MyRecord account.

Q: I forgot my password. What should I do?

A: You may click the "Forgot password" link on the sign-in page to reset your password online.

Q: How do I change my password?

A: Log into MyRecord, select the arrow next to your name to expand the menu, and then select ‘Security Settings’.

Q: How is MyRecord secure?

A: We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal usernames, and passwords. Each person controls his or her password, and the account cannot be accessed without that password. Further, MyRecord uses the latest encryption technology to secure your MyRecord session.

 A: MyRecord is owned and operated by OhioHealth and is fully compliant with federal and state laws pertaining to your privacy. Your name and e-mail address will be treated with the same care and privacy given to your health records and will never be sold or leased by OhioHealth.
