  • Review discharge and follow-up instructions as a reminder of the recommended next steps to continue your recovery
  • Print and take a copy to your follow-up physician visits
  • Keep a record of your visit for future reference

Depending on the type of care you received, your OhioHealth MyChart may give you access to:

  • View some test results online
  • Fill out health forms prior to coming in for a visit
  • View your past and upcoming appointments

For patients who have an OhioHealth primary care or specialist physician, your OhioHealth MyChart also gives you access to:

  • Request non-urgent future physician appointments
  • Request prescription refills online
  • Send a secure message to your physician’s office to get medical advice and answers to non-urgent questions

Activate your Account 

Step 1: Get your activation code

  • Ask the registration, medical receptionist or discharge staff or any member of your care team for your activation code.
  • Or, go to to request your activation code.
  • Your activation code will enable you to log in the first time and create your own username and password.
  • Your activation code is good for 30 days.

Step 2: Activate your account

  • Go to and follow the instructions to log on with:
    • Your email address
    • Activation code
    • Last four digits of your social Security number
  • You will create a password — save this in a secure place

You can also access OhioHealth MyChart from your iPhone or Android device. Download the MyChart app from your app store.

A:  No. This service is free to our patients.

Q:    How do I receive an activation code for OhioHealth MyChart?

A:  If you have received care at most OhioHealth hospitals, outpatient centers, physician offices or urgent care centers, you may already have an OhioHealth MyChart account. To access OhioHealth MyChart for the first time, you need an activation code number. You have 2 options to receive an activation code:

  • Ask the registration, medical receptionist or discharge staff or any member of your care team for your activation code.
  • Or, go to OhioHealth MyChart to request your activation code.
