What should I bring to my first appointment?
Please bring the referral from your physician, your insurance card, a photo ID and a list of any current medications and allergies.

Please arrive 10 minutes before your first appointment to complete intake forms. You can also download the forms and complete them prior to your visit.

What should I wear to my treatment sessions?
If possible, please wear comfortable clothing to your appointment. Exercise clothing will allow your therapist to better evaluate your condition.

How long will I need to go to therapy?
Patients' therapy teams meet regularly to coordinate care and provide progress updates to physicians. How often and how long patients participate in therapy depends upon their treatment plan and goals.

Can anyone receive therapy or enroll in one of OhioHealth's Rehabilitation Programs?
All therapies and rehabilitation programs require a referral from a physician. Talk to your physician to see if the programs at OhioHealth Rehabilitation Services are right for you.
