Outpatient Cardiac Rehabilitation Maintenance

The Cardiac Rehabilitation Maintenance Program at Grant Medical Center offers a medically supervised and structured membership program. This program provides the continued support and structure needed to help you maintain the healthy lifestyle habits developed during Cardiac Rehabilitation.

This self-pay program is tailored to meet your individual needs and is based on a final evaluation from an outpatient cardiac rehabilitation program. To safely and confidently manage your exercise patterns, you will monitor your own pulse rate before, during and after exercise. A care coordinator will work with you to evaluate your progress and to update your program as needed.

You are able to restrict your exercise hours to ensure that qualified emergency staff is present for emergencies. Participants may attend Cardiac Rehab education classes during their membership period at no charge. In addition, Cardiac Rehabilitation Maintenance members have additional educational classes to meet their needs.
