Our diabetes treatment programs at the McConnell Heart Health Center and the Grant Health and Fitness Center in Central Ohio can help you achieve your best level of health through personalized educational services. We provide the most current information and resources for self-management of Type 1, Type 2 and Gestational Diabetes.

Call the Grant Health & Fitness Center at (614) 566-8934. With a referral prescription from your physician, some insurance plans will cover nutrition and diabetes education services offered at Grant. Please check with your provider prior to your appointment date, if you wish to submit services to your insurance provider. Referrals can be faxed to (614) 566-8004.
