Our office is located in the Riverside Health Center just north of the hospital at 500 Thomas lane, 2nd floor, Suite C. Parking is available in the attached garage located behind the center. Parking fees are charged.

Appointments and scheduling
- Please make appointment before you come in. When you call, describe your problem as clearly as you can so that enough time is scheduled to meet your needs.
- 1st visit-please allow about two hours. This gives you time to register, share your health history, and in some cases, meet with a staff nurse before seeing your doctor.
- To help you remember you appointment, a computerized reminder system called SmartTalk will call you one or two days in advance.
- Please arrive 15 minutes before your appointment, or call (614) 566-5456 at least 24 hours ahead of time if you are unable to keep it.

Medical refills
Please bring all medicines to each doctor visit so that refills can be written at that time. This will avoid unnecessary delays in refilling medications. If you must call our office at (614) 566-4060 for a refill, please tell us the following information, which can be found on your prescription bottle:
- The name of the doctor who prescribed the medication
- The name of the medication, and the dosage instructions on how it is to be taken.
- The prescription number
Also give the name and phone number of your pharmacy and your daytime phone number. Allow 48 hours for physician review of your medication request.
For your safety, some medications cannot be filled without seeing a doctor.

Emergency help after hours
- Please call 911 if your life is in danger. You will probably be taken to the emergency department of the nearest hospital.
- For urgent concerns after office hours, call (614) 566-5000 and ask for the resident on call.
