Palliative Care is a consult service that provides comfort care with
emotional and spiritual support for patients and their families with a chronic,
progressive or serious illness or injury.
The Palliative Care Team at Grant works with your doctor to plan care with
the goal of improving your comfort and quality of life. The team helps you and
your family to understand your illness better. They are available to assist you
and your family in making difficult decisions about your care and in dealing
with the stresses of the illness or injury.

Who is on the Palliative Care at Grant Team?
- Physician
- Clinical Nurse Specialist, certified in hospice and
palliative care
- Nurse Practitioner
- Social Worker
- Chaplain
- Pharmacist
Who can receive Palliative Care at Grant Medical Center?
Persons who:
- Have chronic, complicated, or numerous medical
problems
- Have intense or numerous psychosocial, spiritual
and/or caregiver needs
- Are frequently in the emergency department or are
often re-admitted to the hospital
- Are in an intensive care unit and need help deciding
what to do
- Need support with end-of-life decisions
- Have difficulty managing the complexity of their medical problems

- Ease you and your family's concerns and distress
- Help you feel better and improve your quality of life
- Answer any questions that you and your family may
have
- Help you obtain community resources, including
medications if you need them
- Provide a chaplain who will address your spiritual
needs and concerns
- Assist you and your family in making important
decisions about life support treatments and do not resuscitate orders
- Help you plan ahead by completing a living will and health care power of
attorney forms
How can I afford Palliative Care?
Most insurance companies pay for palliative care. It is included as a part of
your hospital bill. You will not be personally billed for these services.

consultation.
